Passport Acceptance Hours: 8:30am-4:30pm
Please allow 30 minutes to verify and complete your application.
Applications are available at the Courthouse or online at www.travel.state.gov. If applying online, print out application on two separate pages. Do NOT print double-sided.
**Applications must be filled out in BLACK INK.
Applicants must appear in person.
If applicant is under 16 years of age, BOTH PARENTS & CHILDREN must be present and provide proper photo ID to sign for child.
If applicant is 16 or 17, one (1) parent must be present with proper photo ID and sign with applicant.
Applicants need to bring the following:
- DS-11 Form (Passport Application)
- A current government issued photo ID
- Certified Birth Certificate or Original Naturalization Paper.
- Hospital birth records, unverified birth certificates are NOT acceptable documents.
- Originals will be sent with the application and returned from the State Department.
- One (1) 2″ x 2″ Passport Photo
- Cash or Check payable to Camas County in the amount of $35.
- A Personal Check or Cashier’s Check payable to the Department of State (see fee schedule below)
Fees:
Passport Book – Valid for travel by air, sea, or land
- $130.00 — Age 16 and older (valid for 10 years)
- $100.00 — Age 15 and under (valid for 5 years)
Passport Card – Valid for land and sea, crossing between the US & Canada, Mexico, Bermuda & the Caribbean ONLY
- $30.00 — Age 16 and older (valid for 10 years)
- $15.00 — Age 15 and under (valid for 5 years)
Departure dates less than four (4) weeks will be required to pay the additional $60.00 Expedite Fee.
Additional Priority Express Deliver options are available:
- $31.40 — Priority Express Delivery to Passport Facility, payable to Camas County
- $21.36 — Priority Express Return* to you, payable to the Department of State
